Weight-based pediatric dosing charts for Acetaminophen and Ibuprofen
Critical Dosing Safety Warning
- Always dose by WEIGHT first: Age ranges are general guidelines. The most accurate dose is calculated by the child's actual weight.
- Use the measuring device provided with the medicine: Kitchen spoons are not accurate. Only use the oral syringe or dosing cup that came with the product.
- Double-check concentrations: Compare the strength of your liquid (e.g., 160 mg / 5 mL) with the bottle label before dosing.
- Never exceed recommended frequency: Acetaminophen is given every 4-6 hours (max 5 times in 24 hrs). Ibuprofen is given every 6-8 hours (max 4 times in 24 hrs).
- Under 3 Months / Under 12 lbs: Always consult a pediatrician before giving any fever-reducing medication.

Acetaminophen Concentration Guidelines: Current liquid acetaminophen is sold in one concentration: 160 mg per 5 mL. Brand names include Infants' Tylenol, Children's Tylenol, or store brands. Always check the label.

Fever Guidance
Fever is a sign that the body is fighting infection. Treatment is recommended if the child is uncomfortable, fussy, or has a very high temperature. When to Call the Doctor
- Your child is under 3 months old with a rectal temperature of 100.4°F (38°C) or higher.
- The fever is persistently above 104°F (40°C).
- Fever lasts more than 3 consecutive days.
- Child is lethargic, has difficulty breathing, or develops a stiff neck or new rash.
